Please review and sign our Terms of Service & Authorization form. For your convenience, you may sign a copy for our records at Check-In if you prefer or are unable to download and print. You will only need to sign once for the duration of services with us, unless there is a lapse in services for more than 1 year.
If your dog is new to our services, you must complete our New Guest Application before making a reservation.
We allow and encourage group play for those who enjoy socializing with others. For the safety of your pet and others, we cannot guarantee that dogs who are not spayed or neutered after 6 months of age can participate in group play. We evaluate at each visit whether or not participation in group play is appropriate and safe for each guest.
Bring an ample supply of your pet’s food for their stay. Maintaining the same diet helps to ensure a comfortable transition from your home to Flat Creek Pet Care and back home again. A change in diet, coupled with being nervous in a new environment, can cause upset tummies! No need to bring food or water bowls as we have plenty. Food stored in its original packaging or in Ziploc bags is best.
Optional: You are welcome to bring your pet’s own toys, bedding and treats. Often something that smells like home is comforting for a pet, particularly if being boarded for the first time. Please remember, we cannot guarantee bedding and toys will go home in the same condition in which they arrived. We are limited on space for storage, so please be conservative with the amount of bedding, toys or storage bins you bring. Keeping items to one flexible tote or a plastic bin. Rooms are approximately 4×6 or 5×5, so bedding needs to fit within that space along with food and water bowls; so very large bedding, though ideal for your home, may be too large for here. If you forget or choose not to bring any bedding, we have extra on hand. We will ensure your pet is comfortable while here!
